This article explains how to set up a Form Stack account which helps your department automate processes of creating digital forms and collecting data for surveys, leads, registrations and more.

There are two account types to choose from:
Temporary License
For users who would like short-term access to create or migrate all of their forms, but forms do not require maintenance after creation. Accounts are removed after 90 days. However, forms are only deleted upon request.
If you have decided that you need a Temporary License, fill out the account request form:

When the account is processed, you will receive a direct email from Formstack with details on creating your login credentials.
Once the account is finalized, you will have access to a folder that will hold all of the forms you need to create.
Formstack Account
Annual charge for use. For users with a high volume of form needs/creation and users whose forms include document upload options. This account type is renewed annually and users have full access to create and edit forms as needed.
