This article explains how to add a form from Formstack to your Cascade site.

To add a form to your Cascade site:

  1. Open Formstack and select the form you would like to use.

  2. Navigate to the Share tab at the top of the screen.

  3. There are two ways to include a form on your website:

    • Link to the form within Formstack – this will take the visitor away from your site to allow them to fill out the form directly in Formstack

    • Embed the form within a Cascade Page – this allows the visitor to complete the form without leaving your NMSU site.

  4. To add a link to your form in Cascade, copy the direct link from Formstack and link it to a word within your desired Cascade page. (see article Types of Links)

  5. To embed the form into your Cascade site, copy the embed code from Formstack, and paste it into the Source Code section of your WISIWIG on the desired Cascade Page. (See article How to Embed a PDF.)

Please note, you can make changes to your form in Formstack, and the changes will be reflected on your current link or embedded  form. HOWEVER, if you change the URL of your form you will need to follow the above steps to re-embed the form or re-link it onto your website.
